Obese Women at Higher Risk of Breast Cancer Recurrence: Study ...


FRIDAY, March 23 (HealthDay News) ? Overweight and obese breast cancer patients are at increased risk for recurrence of the disease, a new study finds.

The findings ? which held true even though chemotherapy doses were adjusted for weight ? provide further evidence that lifestyle factors can influence cancer patient outcomes, according to Dr. Jennifer Ligibel, a medical oncologist at the Dana-Farber Cancer Institute in Boston, and an associate professor at Harvard Medical School.

She and her colleagues analyzed data collected from 1,909 breast cancer patients. Of those women, about 1 percent were underweight, 33 percent were normal weight, 33 percent were overweight and 33 percent were obese.

The researchers examined the link between body-mass index (a measure of body fat based on weight and height) and relapse-free survival and overall survival. The findings are slated for presentation Friday at the European Breast Cancer Conference in Vienna.

?Several other studies have shown that being overweight or obese at the time that a woman is diagnosed with breast cancer is linked to a higher risk of recurrence. However, questions have been raised in the past whether obese women were receiving relatively lower doses of chemotherapy due to their weight. Our study mandated that each patient received a chemotherapy dose adjusted to her weight, so these results suggest that treatment factors are not responsible for the differences in recurrence rates seen in heavier women,? Ligibel said in a conference news release.

?We found that BMI was related to both relapse-free survival and overall survival; for example, the 10-year relapse-free survival of a patient who was overweight was 70 percent; compared with 65 percent for one who was obese,? she noted.

Ligibel concluded: ?Obesity is a modifiable factor, and although there is not yet enough evidence to say with certainty that losing weight or exercising more regularly will decrease the risk of breast cancer recurrence, there are consistent links between lifestyle factors such as diet, weight and physical activity patterns and breast cancer prognosis. If future studies show that making changes in lifestyle behaviors for women with early breast cancer will improve survival rates, then lifestyle interventions may one day become a standard part of breast cancer care.?

According to conference chair David Cameron, a professor at the University of Edinburgh in Scotland, these are important findings for women with breast cancer but ?we need to recognize that the reason overweight women have poorer outcomes is not clear,? he said in the news release.

?There are a lot of health reasons why overweight women should try and get back to a normal weight, but this is not always easy, and as the authors acknowledge, we don?t yet know that losing weight after a breast cancer diagnosis will make a difference,? he noted.

Because this study was presented at a medical meeting, the data and conclusions should be viewed as preliminary until published in a peer-reviewed journal.

Featuring a wide-ranging list of musicians including Neko Case, Arcade Fire, Maroon 5 and Kid Cudi, the album's themes and sounds pay homage to the story's subject matter of rebellion and war, as well as the film's setting in a dystopian version of the Appalachian Mountains, thanks to lots of twangy guitars and a handful of country ballads.

"And what excited me about it was it wasn't your typical young adult romance. There was some dark underlying things to it, some pretty heavy situations. I just fell for the plot and it was obvious why other people did too. I think it actually earns it [and all the attention]. I don't think we knew the size of it until doing this and realized how rabid people are for this first release."

For their work with Swift on "Safe," they knew they wanted to draw out a darker side of the normally optimistic singer.

"We had all read all the books and we were all fans of all the books and I think we knew with us being a part of the equation that it was probably going to be a little darker than she normally does, which she was fully a part of the idea," White explained.

"The books are not the cheeriest. It's not a fairy tale. That's what we loved about the books," he continued. "We talked about characters we liked and relationships that we dug, but it all kept leading back to some sort of lullaby. But we didn't want you to know who's talking to who. There's probably four different relationships in the books that can fit that theme. Even if you've never read the books, you can make it have some sort of parallel in your life as well, and that was the priority for us."

And when it came to their solo contribution, Civil Wars' other half, Joy Williams, explained that they didn't stray too far from that philosophy. "T-Bone [Burnett] called us up ... we both jumped on [the opportunity] having been aware of the book. Out came this sort of, in my opinion, it sounds like this quiet survival song and almost this mantra you sing over to yourself. I'm excited about [the song and the soundtrack] and excited about the company [we keep on it]. I think the soundtrack's going to be really solid and we're just stoked to be a part of it."

With roughly 1.5 million school-age children in this state, that means nearly a half-million are on track to experience problems associated with obesity: diabetes, hypertension, fatty liver disease, even heart disease and cancer.

Even those who survive their bouts with such diseases face a lifetime of health complications, multiple doctor visits and use of prescription drugs and therapies to manage their conditions. As grownups, their career options and ability to enjoy things such as sports and travel vacations will be limited.

It doesn?t have to be that way, however. Most obese children can reach a healthy weight if they begin to get a balanced, nutritious diet, save sugary treats for special occasions, and get regular exercise.

We know the goals; now we must find out how to get there. Unlike adults who struggle with obesity, kids don?t control their food sources. Depending on where they live, they may have few options for exercise. So, while the message about child obesity must first hit home with parents, there is a role for government to play.

Many people hate to hear that, but it?s true, because the world has changed. Many Tennessee kids live in single-parent households or have two parents working multiple jobs, where cooked family meals rarely occur. Many live in low-income neighborhoods ? rural as well as urban ? with a single grocery that stocks primarily processed foods because they have a long shelf life, and a number of fast-food restaurants. In neighborhoods where crime is high and patrols are low, kids are less likely to go out and play.

So be glad when Gov. Bill Haslam announces that the state is working on ways to lower child obesity. His Health and Wellness Task Force plans to build on the work of groups including the Tennessee Obesity Task Force, the YMCA and the Metro Public Health Department.

And be glad that the U.S. Department of Agriculture has begun requiring public-school cafeterias nationwide to cut sodium, use more whole grains, and serve low-fat milk.

Not all government-centered solutions are a good idea. A tax on soft drinks that contain sugar, for example, would be unlikely to make a dent in kids? consumption and would unfairly hit low-income people the hardest.

But where government agencies can help is to bring healthier food choices to cafeterias and school vending machines; fund improvements in public playgrounds and ballfields and make them safer; and support public-service programs and ads that teach children and their parents about the necessity of healthy food and physical fitness.

University of Chicago physicists have experimentally demonstrated, for the first time, that atoms chilled to temperatures near absolute zero may behave like seemingly unrelated natural systems of vastly different scales, offering potential insights into links between the atomic realm and deep questions of cosmology.

This ultracold state, called "quantum criticality," hints at similarities between such diverse phenomena as the gravitational dynamics of black holes or the exotic conditions that prevailed at the birth of the universe, said Cheng Chin, associate professor in physics at UChicago. The results could even point to ways of simulating cosmological phenomena of the early universe by studying systems of atoms in states of quantum criticality.

"Quantum criticality is the entry point for us to make connections between our observations and other systems in nature," said Chin, whose team is the first to observe quantum criticality in ultracold atoms in optical lattices ? a regular array of cells formed by multiple laser beams that capture and localize individual atoms. Chin, UChicago graduate student Xibo Zhang and two co-authors published their observations online Feb. 16 in Science Express and in the March 2 issue of Science.

Quantum criticality emerges only in the vicinity of a quantum phase transition. In the physics of everyday life, rather mundane phase transitions occur when, for example, water freezes into ice in response to a drop in temperature. The far more elusive and exotic quantum phase transitions occur only at ultracold temperatures under the influence of magnetism, pressure or other factors.

"This is a very important step in having a complete test of the theory of quantum criticality in a system that you can characterize and measure extremely well," said Harvard University physics professor Subir Sachdev about the UChicago study.

Physicists have extensively investigated quantum criticality in crystals, superconductors and magnetic materials, especially as it pertains to the motions of electrons. "Those efforts are impeded by the fact that we can't go in and really look at what every electron is doing and all the various properties at will," Sachdev said.

Sachdev's theoretical work has revealed a deep mathematical connection between how subatomic particles behave near a quantum critical point and the gravitational dynamics of black holes. A few years hence, offshoots of the Chicago experiments could provide a testing ground for such ideas, he said.

There are two types of critical points, which separate one phase from another. The Chicago paper deals with the simpler of the two types, an important milestone to tackling the more complex version, Sachdev said. "I imagine that's going to happen in the next year or two, and that's what we're all looking forward to now," he said.

Wilson Distinguished Service Professor in Physics at UChicago, observed quantum criticality in a sample of pure chromium when it was subjected to ultrahigh pressures.

Zhang, who will receive his doctorate this month, invested nearly two and a half years of work in the latest findings from Chin's laboratory. Co-authoring the study with Zhang and Chin were Chen-Lung Hung, PhD'11, now a postdoctoral scientist at the California Institute of Technology, and UChicago postdoctoral scientist Shih-Kuang Tung.

In their tabletop experiments, the Chicago scientists use sets of crossed laser beams to trap and cool up to 20,000 cesium atoms in a horizontal plane contained within an eight-inch cylindrical vacuum chamber. The process transforms the atoms from a hot gas to a superfluid, an exotic form of matter that exists only at temperatures hundreds of degrees below zero.

"The whole experiment takes six to seven seconds and we can repeat the experiment again and again," Zhang said.

The experimental apparatus includes a CCD camera sensitive enough to image the distribution of atoms in a state of quantum criticality. The CCD camera records the intensity of laser light as it enters that vacuum chamber containing thousands of specially configured ultracold atoms.

"What we record on the camera is essentially a shadow cast by the atoms," Chin explained.

The UChicago scientists first looked for signs of quantum criticality in experiments performed at ultracold temperatures from 30 to 12 nano-Kelvin, but failed to see convincing evidence. Last year they were able to push the temperatures down to 5.8 nano-Kelvin, just billionths of a degree above absolute zero (minus 459 degrees Fahrenehit). "It turns out that you need to go below 10 nano-Kelvin in order to see this phenomenon in our system," Chin said.

Chin's team has been especially interested in the possibility of using ultracold atoms to simulate the evolution of the early universe. This ambition stems from the quantum simulation concept that Nobel laureate Richard Feynman proposed in 1981. Feynman maintained that if scientists understand one quantum system well enough, they might be able to use it to simulate the operations of another system that can be difficult to study directly.

For some, like Harvard's Sachdev, quantum criticality in ultracold atoms is worthy of study as a physical system in its own right. "I want to understand it for its own beautiful quantum properties rather than viewing it as a simulation of something else," he said.

Johnson rallies Kansas to 63-60 win over Purdue

(AP) ? The messages were flying in the Kansas locker room at halftime.

Elijah Johnson told his team to stop second-guessing itself. Tyshawn Taylor reminded the Jayhawks that they'd been in trouble before. Coach Bill Self finally stood up and told his team that it would take sheer toughness to rally past Purdue.

"When things don't go well and you get behind and you play uphill the whole way, it takes some energy and it takes some toughness," said Self, whose team fell behind by double-digits early and still trailed by six at the break. "My message to the whole team was trust each other."

All those messages came through loud and clear.

Johnson finished with 18 points, including the go-ahead basket in the final minute, and No. 2 seed Kansas held off Robbie Hummel and the No. 10 seed Boilermakers 63-60 on Sunday night in a dramatic third-round game in the NCAA tournament.

"I felt it would come down to one possession no matter what," Johnson said, "the whole time."

Purdue was clinging to a 60-59 lead and had the ball and under a minute remaining when Lewis Jackson, the shot clock winding down, lost control at the top of the key. Johnson picked it up and went the other way for the go-ahead lay-in with 23.3 seconds left.

Hummel missed an open 3-pointer at the other end and Taylor scored a transition dunk for the Jayhawks with 2.5 seconds left, giving the roughly 15,000 fans who had made the three-hour drive from the Kansas campus reason to let out a roar for one of the first times all night.

After a timeout, Purdue sharpshooter Ryne Smith managed to get off a decent look at a long, potential tying 3-pointer. It hit off the backboard, clanked off the rim and finally fell away.

The Jayhawks leaped in jubilation. The Boilermakers sank in defeat.

"It stinks," Purdue coach Matt Painter said. "It stinks to lose."

Thomas Robinson fought through double-teams all night for 11 points and 13 rebounds, and the Jayhawks (29-6) got enough production from everyone else to erase a 10-point second-half deficit and reach the Midwest Regional semifinals in St. Louis.

Kansas will face No. 11 seed North Carolina State.

"What a great game. It wasn't the best played, but it was a grind-it-out, typical Big Ten game," Self said. "Hummel was unbelievable and we just hung in there."

Hummel finished with 26 points and nine rebounds for the Boilermakers (22-13), who were trying to reach the round of 16 for the third time in four years. D.J. Byrd and Terone Johnson finished with 10 points each for Purdue.

The Jayhawks' biggest lead all night was their final one. They overcame a ragged night from Robinson by getting 10 points from Taylor and 10 more from Travis Releford.

"That isn't a team loaded with just four or five guys," Painter said.

Purdue couldn't have gotten off to a much better start, and neither could Hummel, who hit three 3-pointers while making his first four shots. The senior forward, who has come back from two torn ACLs in his right knee, even followed his first miss with a basket to make it 19-8.

He proved too quick for Robinson to guard and too strong for Kevin Young as the Jayhawks kept searching for anybody who could put a body on him ? they even tried seldom-used Justin Wesley.

The miserable start by Kansas was enough for Self to scream at his team during one defensive trip down floor, "You told me you were ready!"

Hardly seemed to be the case.

Kansas opened the game by missing 15 of its first 17 shots and all seven of its 3-point tries, compounding lousy offense by getting into foul trouble. Taylor, Young and Releford all sat stretches in the first half after picking up two early fouls.

The Jayhawks finally trimmed the lead to 31-30 with under 3 minutes left in the first half, but Lewis Jackson got inside for a basket, and Hummel managed to swish a closely guarded 3 from about 30 feet as the shot clock wound down to make it 36-30 at the break.

Hummel had 22 points on 7-of-8 shooting in the first half, while the Jayhawks' trio of stars ? Robinson, Taylor and Johnson ? managed 12 points on a combined 4 for 18.

"I wanted to come out and be aggressive, especially the first half. It seemed like everything I was taking was going in," Hummel said. "It was a crazy feeling you have as a player."

Purdue extended the lead to 42-32 early in the second half, even after Kansas employed a zone defense to slow down Hummel. Johnson led the charge on offense, and the Boilermakers kept locking down Robinson in the post, frustrating the player of the year candidate to no end.

Kansas never went on its patented run, instead slowly clawing back into the game.

The Jayhawks trimmed the lead to 47-44 midway through the second half, but came up empty with four open shots on offense. They got within 52-49 minutes later only for Taylor to turn the ball over. And it was 52-51 with 5? minutes left when Hummel drove for a layup high off the glass.

Kansas never led until Johnson hit a deep 3-pointer with just over 3 minutes left to make it 57-56. Terone Johnson answered with back-to-back baskets for Purdue to regain a 60-57 lead, but Taylor's alley-oop jam off a feed from Elijah Johnson made it a one-point game.

And set up a dramatic final flurry between Kansas and Purdue.

"We just kept grinding and grinding," Taylor said, "and we ended up making some big plays down the stretch."
